If you have relevant medical information at another practice, we can organise for your transfer of records by completing a transfer of history form. The reception staff can forward the request to your previous doctor on your behalf.

Your first consultation will require a 30 minute appointment. During this consultation the doctor will collect information about your past history and current health status. All information gathered is treated with the strictest confidentiality and is used to allow our doctor to provide the best care for you.
